YOUR FIRST CHIROPRACTIC VISIT

New Patient Forms

​To ensure that you receive the best possible care and more time at your first appointment, please fill out the forms below that are specific to your needs and bring them with you. If you don’t have time or don’t have computer access with a printer, the forms are available at the time of your visit.

Please note any current medications on your paperwork. Also be sure to bring with you any current imaging for your condition, such as X-rays or MRI studies. 


About 80% of patients do not require X-Rays prior to treatment.

IMPORTANT - Note specifically what makes your pain better and worse. Also note how the pain has changed since the condition started. This information may be important for your treatment.

HIGH QUALITY CARE TAKES TIME

Spending more time with patients is one of the most important things that makes us different. Dr. Morris takes time to listen and learn about your needs. He takes time to examine you thoroughly and consider all effective treatment options. He takes time to reassess your progress and needs as you get better and start your journey towards a healthier lifestyle.
Your entire visit (20-25 minutes) will be with Dr. Morris.

WHAT TO EXPECT DURING YOUR VISIT

Your First Chiropractic Visit

When you arrive at your appointment time Dr. Morris will greet you personally and review your paperwork immediately. You won't sit in a crowded waiting room or alone in an exam room.

Please tell the doctor if your condition is a Worker’s Compensation Case or a Motor Vehicle Accident claim on the first visit. These situations may require more documentation.

Dr. Morris will take your history and review any medications or supplements you are currently taking. You will review previous treatments and approaches so you aren’t repeating the same treatment to no effect.

Next you will have a Physical Exam that includes a Review of Systems, which is a general look at your health as a whole person (pulse, blood pressure, breathing, etc.) then a more focused exam of your problem area(s). The neck, mid and low back can have problems with the associated joints, muscles, tendons and disks of each area; possibly a combination of causes. Some of these areas respond best to chiropractic adjustments. Some do not. The treatment approach will be different for each root cause of your problem.

It is believed that almost half of back pain is caused by disc pain. There is no better manual examination for disc pain diagnosis than using the McKenzie Method.

McKenzie Exam

The McKenzie Method is internationally recognized for assessing, treating, and preventing  back pain and radiating extremity pain. You work closely with Dr. Morris to identify the specific movements and postures that are effective for your individual pain. The goal is to cause the pain to centralize (move from your extremities toward the center of the spine) and then resolve.

With the help of a trained McKenzie clinician, patients with back pain that responds to McKenzie therapy are very likely to resolve their back pain in a matter of days, rather than weeks or months.

PAYMENT & INSURANCE

Insurance
Advanced Spine Care & Wellness is a cash-based practice, meaning that we do not routinely process insurance, except in the case of worker's compensation and motor vehicle accidents. We prefer to eliminate the high costs associated with insurance claims and pass that savings on to patients. If you would like to submit your own insurance claim, we will gladly provide you with your records. 

Payment
We gladly accept cash, check, and credit cards with no added fee.

AFTER YOUR CHIROPRACTIC APPOINTMENT

You may receive a follow-up call from Dr. Morris.
He cares about your well-being and helping you return to optimal health. He will ask if you have any concerns or questions and note any changes in your condition since your appointment.

Do your Prescribed Exercises!
Prescribed exercises are your body’s natural “medicine.” If you don’t do them as described, as often as prescribed, it's likely that your condition will not improve as expected. Call or text Dr. Morris if you have questions about your exercises or ask them at your next visit to ask your questions.

Does everyone need continued treatment?
Ongoing treatment is not required if you don’t want it or if it's not needed. That said, there is well documented medical literature supporting the effectiveness of maintenance care in patients with chronic low back pain. Be sure to ask Dr. Morris if a maintenance plan makes sense for you.

Exercise, Diet and Lifestyle, Supplements
Be sure to ask if there are any supplements, additional exercises or lifestyle modifications that would help your condition and support your overall health. Important health choices include stopping smoking, committing to regular exercise, and finding healthy alternatives to soda and other high sugar foods and drinks. Chiropractors have many hours of graduate nutrition training and in many cases are more knowledgeable and up to date than medical doctors about nutritional wellness topics.
